4. New eform Offering Forms that can be embedded on CMS pages
5. eforms Prerequisites • Do not have Personally Identifiable Information (PII) on the form • No payment processing • Have a CMS website
6. Personally Identifiable Information (PII) is not allowed PII information is: • First name or first initial and last name in combination with • SSN • Driving license number/ID number • Account number and any record that will give access to financial, health and personal data. (more information on CWO website)
