50. ADMINISTRATIVE SECURITY AND PROCEDURAL REQUIREMENTS 1. Provider Administration, initially, must provide Approved list of Authorized Providers 2. All Providers on initial list will not be required to follow steps 4 - 6 3. Provider Administration can assign, in writing, an “Add and Approve List” Coordinator(s) 4. Provider Must Register, Confirm Email, Await Approval 5. Notification will be sent from BelleIT for approved SmartViewMD access 6. Provider will be required to reset their Password 7. All Providers will begin receiving weekly DSM emails to screen and attach to EHR 8. All Providers will have immediate access to their Patient portal to view patient information 9. HR to provide written notification to BelleIT of terminated employees upon exit interview
51. ADMINISTRATIVE SECURITY AND PROCEDURAL REQUIREMENTS 10. Provider Administration should assign a coordinator to assist new patients with application instructions and medical device use 11. Patients to call BelleIT customer service number with software issues or any issues after coordinator training at normal business hours posted on the SmartView MD webpage 12. Assigned administrative coordinator should contact Belle IT, in writing, of any patient that requires removal from SmartView MD 13. Provider software issues or questions that arise may contact the BelleIT Office number on our SmartView MD webpage for immediate assistance 14. Provider Administration (CEO, CMO) will have view of ALL SmartView MD enrolled patients in their Provider Portal view 15. Grant related Custom Graphs and unlimited software upgrades can be added, within the initial 3 year window of SmartViewMD purchase and only for Tier 3 customers
