  1. CLIENT CONSENT FORMS Having your clients fill out a consent

    form is crucial. It may not seem like you need one for a service like brow threading, but your consent form will have the opportunity to gather key information about your client and their relevant medical history. This is where you'll ask about the products they're using, supplements they're taking, medications they're on — these are all important details you'll need to know to assess whether or not they are a suitable candidate for the service you're going to perform. Your insurance company will likely require that you have a client consent before they offer coverage, and your municipality or Public Health office will also ask that you have these on hand for a certain amount of time.
  2. CLIENT CONSENT FORMS At my studio, I have client consent

    forms done digitally and stored in an online database. It's a good idea to have a searchable database so that you can look through your client's information prior to their arrival if they're a repeat client, and verbally confirm that nothing on their consent has changed. If they're new, you can review it before signing off an submitting it to your database. You can also save time checking new clients in by sending them their client waivers beforehand. There are many applications available for you to use, mostly paid. If you're looking for a free option until your business grows to the point where you need a digital platform, paper waivers will do just fine. The downside to paper, aside from environmental impact, is the physical space it takes to store client files as well as the inability to use a search function to look for an existing client.
  3. CLIENT CONSENT FORMS The next section is a downlodable consent

    form that you can use as a guide. This is the form I have loaded into our waiver application, and is completed digitally by clients prior to their first appointment. Our specific platform allows clients to use their finger or stylus to sign off and initial specific areas, and has tickboxes that clients can check off to indicate medications, symptoms, etc. Review the form and edit as necessary. You can also choose to make up your own waiver, but check with your insurance and municipal health officials to ensure your waivers meet their requirements before beginning to use them.
