  1. PRACTICAL CARDIAC GUIDE Breathlessness: Understanding Causes & When to Seek

Help A practical guide to cardiac and non-cardiac causes, assessment and urgent warning signs. HEART HEALTH General educational information • Not a diagnosis or personal medical advice CARDIAC HEALTH 01 / 06
  2. 01 • THE SYMPTOM Breathlessness is a symptom, not a

diagnosis Shortness of breath can have many causes, including cardiac, respiratory and other conditions. Look at the pattern Breathlessness may appear during exertion, at rest, when lying down or in particular situations. The cause can be straightforward, but persistent or worsening breathlessness should be assessed. When symptoms started, what brings them on, how quickly they settle and what other symptoms occur can all help guide assessment. Heart-related possibilities can include heart failure, coronary disease, valve problems and rhythm disorders. Respiratory disease, anaemia, deconditioning and anxiety can also contribute. 3. 02 • CLUES TO NOTICE The context around breathlessness matters

A structured history can help separate different possible causes. With activity At rest or lying down Alongside other symptoms Note whether ordinary walking, stairs or previously manageable activity now causes unusual breathlessness or requires more frequent rests. Breathlessness at rest, when lying flat, or with new swelling can warrant prompt medical assessment, depending on the overall picture. Chest discomfort, palpitations, fainting, coughing, wheeze or leg swelling can provide important clues about what needs checking. CARDIAC HEALTH 03 / 06
  4. 03 • POSSIBLE INVESTIGATIONS Assessment may involve more than one

test There is rarely a single test that explains every case of persistent breathlessness. A broader view Clinical assessment may include medical history, examination, blood pressure, pulse and oxygen saturation. Depending on the situation, an ECG, blood tests, chest imaging or lung-function testing may be considered. Breathlessness can be multifactorial. The most useful assessment considers the heart, lungs, general health and functional capacity together. An echocardiogram can be useful when a cardiac cause is suspected. If the initial assessment does not explain symptoms, further specialist investigations may be appropriate. CARDIAC HEALTH 04 / 06
  5. 04 • SAFETY When breathlessness is an emergency Severe or

rapidly worsening breathing difficulty needs urgent help. UK emergency advice Call 999 if you are severely struggling to breathe, gasping, choking or unable to speak normally because of breathlessness. Emergency help is also appropriate with severe chest tightness or pain, sudden confusion or collapse. If severe symptoms are happening now, call 999. For non-emergency concerns, NHS 111 or a GP can advise on the next step. Sudden or unexplained worsening should not be self-diagnosed from an online resource. For persistent but non-emergency symptoms, arrange appropriate medical assessment. CARDIAC HEALTH 05 / 06
