    room. With a single glance at his Health Diary, you know him better than himself. Yet that’s not all: when you need to learn more about your patient, you tap your way into his medical history.   3. A diary available everywhere Whether it’s a good ol’ desktop

    computer, a tablet, a mobile phone or just paper, the Health Diary adapts itself to the device.
  4. A timeline of your life Every illness finds their roots

    in situations that happened years ago. Want to know more about your health problems from a specific time? The timeline reveals each chronic or unique conditions you had.
  5. See your problems Each problem is located on your avatar.

    It evolves along with your life. Click on the colored body parts to get access to more details.
  6. Find every detail, your way Use keyword in the search

  7. You’re not the same as when you’re born Different periods

    in life require different focus: Immunization at early ages, palliative care at the end. The Health Diary divides the life in 4 stages: Baby, child, adult, and senior. Each period displays a list of milestones. You know when to have a follow-up and get your immunization shots.
  8. Personal information, hidden in plain sight Your information is described

    in a precise, yet clear manner. Icons are used to explain abstract concepts. The diary is color-coded to match with the patient’s medication color-coding.
  9. Optimized for everyone The Diary is color-blind aware. Hand-picked high-contrasted

    colors make it easy to read for people with sight issues.
  10. Detailed patient profile Period selector One line per condition Start

    date, status and occurrences are listed in the timeline for each condition Guardian and providers information Filtering by condition name, doctor name, or any keyword Foldable timeline Previous and next follow-ups Easily slide through the graph When no condition is selected (now), a summary of the patient current health status is displayed

    The interface includes a full log of her hospitalization history, displayed in a clear manner Using the selector, the doctor focuses on specifics, like plan of care only. Harmful events are highlighted
  14. The Health Diary can be flipped through really quickly Depending

    on the focus, the patient’s file or data is displayed. A quick filter enables jumping to a particular section with a tap.
  15. Press the back in time button, and your diary slowly

    moves backward to remind you of your life, one bump at a time. Always wanted to watch your life like a movie?
