4.  Skilled Nursing Hearts & Hands provides each resident with a resident care plan. This care plan is developed to assist each resident in achieving optimum independence in functional abilities and daily living skills. The resident care plan will focus on reducing medical complications and minimizing the need for aggressive medical support and resources.  Post-Acute Rehabilitation Each of our residents comes to us with very different and specific rehab needs. Our in- house, highly skilled physical, occupational and speech therapists provide all the guidance and restorative treatment needed to return your loved one to their highest level of functioning and independence.  Outpatient Therapy Our post-acute care is for those residents who are medically fragile and require additional attention. We seek to maximize the quality of life for our post-acute care population and offer them specialized restorative services. Our goal is to maintain or improve each resident’s highest functional ability.
5.  Long Term Care Long-term care is provided in different places by different caregivers, depending on a person's needs. Most long-term care is provided at home by unpaid family members and friends. It can also be given in a facility such as a nursing home or in the community, for example, in an adult day care center.  Palliative Care Hearts & Hands palliative care program treats residents suffering from serious and chronic illnesses. Our care focuses on helping each resident gain the strength needed to carry on with daily life. We strive to facilitate relief from the symptoms and stress of the serious illnesses our residents suffer with.  Activities Program A positive and uplifting atmosphere is vital to the success of healing, recovery, and rehabilitation. Hearts & Hands employs outgoing and friendly professional staff to create social programs and activities to fit the residents’ needs. We provide a monthly activity calendar to each resident to keep them informed of upcoming events.
6.  Social Services Social services provide essential information, manages requests and concerns, coordinates appointments, maintains contact and open communication between the facility, families and the residents, and assists with care planning and discharge planning for each resident.  Wound Care Hearts & Hands is proud to offer a wound care program with an integrated system of professional, clinical and technical resources designed for the treatment of a variety of wound conditions including ostomies (colostomy/urostomy), wound dehiscence, diabetic arterial/venous ulcers, pressure ulcers, and pre- and post- flap surgery.  Pain Management Pain may be anything from a dull ache to a sharp stab and can range from mild to extreme. You may feel pain in one part of your body or it may be widespread. Pain is a very common condition. The occurrence of pain rises as people get older, and women are more likely to experience pain than men.
