DC completes and fax’s Clinical Care Coordinator Agreement to (855) 771-9109. This establishes the legal relationship of the agreement and outlines the compensation. Step (2) Two: DC or staff distributes Patient Survey forms as part of Standard Case Management to all patients visiting the clinic for routine care and treatment. Step (3) Three: DC completes CMP Form on each patient where a Co-Managing Physician relationship needs to be established (Patients being referred to an MD, DO, DPM, PA or NP). These CMP Form’s are then forwarded to the MD, DO, DPM, PA or NP that they have employed with their Integrated Practice. Step (4) Four: Physician employed with Integrated Practice completes “For Physician Office Use Only” portion of Patient Survey, which assigns the Related Health Problem (ICD-9 or ICD-10) for reporting used in clinical research. Step (5) Five: “For Physician Office Use Only” portion of Patient Survey is then used to append CCC Activity Log and faxed to (855) 771-9109 or emailed to [email protected] every Friday. That’s it.
